How Advocate Health Network, LLC Works For You

Physicians and St. Joseph’s/Candler are teaming up to provide better coordinated care for Medicare patients. The St. Joseph’s/Candler Advocate Health Network, LLC was created to be a patient-centered organization where patients and providers are true partners in care decisions. Advocate Health Network (AHN) is committed to providing high-quality care, streamlining its services and ensuring patient satisfaction and continually improving across each area. A goal of Advocate Health Network is to provide more coordinated, satisfactory care for patients that:

  • Reduces the amount of repetitive paper work you have to fill out

  • All your health care providers  know what’s going on because they communicate with each other

  • Reduces repetitive medical tests because  your results are shared among your health care team

  • Participating physicians will work with you to make sure their health care decisions reflect your preferences

Advocate Health Network currently includes more than 550 physicians and is continuously enrolling new providers. The Network’s physician participants represent both employed and independent physicians, with strong emphasis on primary care and coverage of key specialists who have a history of working collaboratively. Physicians must be a participating provider with The Care Network and maintain medical staff privileges at the in-network facilities, St. Joseph’s Hospital and/or Candler Hospital. Providers also must furnish their own EMR. The Network will provide the interfacing mechanics and incur any costs associated with the interface.

Why would you want to participate in Advocate Health Network?

  • Participating providers in AHN will be eligible to receive a shared payment if the Accountable Care Organization (ACO) meets the quality performance standards and generates shareable savings under the performance-based payment methodology described in the rules of the Affordable Care Act. Shared Savings do not come from reduction in provider fee-for-service reimbursement.

  • AHN will serve as a vehicle for physicians to participate in Alternative Payment Models and qualify for benefits afforded by the MACRA exemption.

  • Clinical Care Support Programs

  • No upfront membership fee is required. Physician membership fees typically range from $500-$1,500 and are taken from shared savings, meaning if there are no shared savings, there are no fees.
